Docebo Learning Impact allows Superadmins and Power Users to enable ready-to-use questionnaires (initial and follow-up questionnaires) to users enrolled for individual courses in your platform, to measure the learning impact of courses. These questionnaires are displayed at the end of the training course.
As these ready-to-use questionnaires have a standard format, you can request the creation of multiple questionnaire templates for both the initial and follow-up questionnaires, during the implementation of Docebo Learning Impact on your platform or later.
In order to request the creation of new questionnaire templates, please contact Docebo using your platform’s Help Center. You can request as many questionnaire templates as you want.
This article will help you in understanding the different questionnaire templates you can request and on how to assign these new questionnaire templates to training courses.
Requesting the Creation of New Questionnaire Templates
To request the creation of custom questionnaire templates dedicated to specific courses and/or for specific audiences of a course, please contact Docebo using your platform's Help Center.
For example, you can request the creation of:
- An initial evaluation questionnaire template dedicated to the Power Users attending the Compliance E-learning course
- An initial evaluation questionnaire template dedicated to the Admins attending the Compliance E-learning course
- An initial evaluation questionnaire template dedicated to all learners attending the Customer Education course
You can also, for example, request the creation of multiple questionnaire templates for the initial evaluation questionnaire, dedicated to learners attending an onsite course and rely just on the default questionnaire template for the initial evaluation questionnaire, dedicated to learners attending an online course.
Once the questionnaire templates you requested have been created, a course additional field is also created in the courses of your choice. Add a value in the additional field, as described in the chapter Assigning a Questionnaire Template to a Course below, in order to assign the questionnaire templates to the courses you want.
Course additional fields are used to provide standard information about your courses in online and onsite courses or to trigger the display of an additional information/file in a course. More details about course additional fields.
Different values are added to the course additional field, which provides you with an assignment rule so that you know which value corresponds to each questionnaire template.
You must validate this assignment rule with Docebo, using your platform's Help Center, before going further.

Note that we can create for you as many assignment rules as necessary to assign Questionnaire Templates to the different courses.
Then, you will just have to set the additional field in the course in order to select or fill out the value corresponding to the questionnaire template you need to assign to the course.
Assigning a Questionnaire Template to a Course
To assign a questionnaire template to a course, you must fill out or select a value in the questionnaire template additional field with the appropriate value (remember the assignment rule), in the course properties.
Reach the Admin Menu from the gear icon in the top right corner. Find the E-Learning section, then press the Course Management item. On the main course management page, click the Courses tab. In the Courses list, find the course you want to assign a questionnaire template to and click on its description to enter the course configuration area, then move to the Properties tab and finally to the Course Additional Fields section.
In the questionnaire template additional field, fill out or select the value that will assign the right questionnaire template to the course and click Save Changes. Refer to the chapter Assigning Additional Fields to Courses, in the article of the Knowledge Base about course additional fields.
